The young patient had been suffering from persistent low back pain, bilateral lower limb pain, tingling, numbness, weakness, and urinary urgency for several years. Detailed evaluation revealed a massive D9-L3 intradural spinal tumor, later confirmed on histopathology as a Myxopapillary Ependymoma (CNS WHO Grade II).
The surgery involved an intricate six-level laminotomy followed by laminoplasty, enabling complete en bloc excision of the tumor while preserving spinal stability and neurological function. The procedure demanded extraordinary microsurgical precision, meticulous planning, and seamless multidisciplinary coordination.
A Rare Achievement in Global Neurosurgery
According to an extensive review of published world literature, only three to four cases of en bloc excision of giant intradural spinal tumors have been reported globally. Of these, only two published cases involve tumors larger than 10 cm in the cervical-thoracic spine, while no published report describes an en bloc excision exceeding 12 cm in the dorsolumbar region.
This remarkable case therefore represents one of the largest successfully excised dorsolumbar intradural tumors reported worldwide, placing NewEra Hospital among the few centers globally to accomplish such a feat.
