NMDA receptor encephalitis associated with super-refractory status epilepticus – the treatment of this disease is as complicated as sounds. 31-year-old Patient Sarojben was brought to the emergency department of GCS Hospital on 13th October with Seizures, aggressive behavior, fever, forgetfulness, and auditory hallucinations.
Dr. Apara Kothiala (Consultant-Neurologist) and Dr. Vipul Prajapati (Asso. Professor-Medicine) assessed her and her past medical history revealed that she had previously undergone surgery for an ovarian tumor which is associated with this disease. Her condition was further complicated by dengue fever.
Then Sarojben was shifted to the ICU for further treatment and close monitoring with ventilator support. Despite all anti-epileptic and high-dose anesthetic medication, there was no improvement in her condition. The family lost their hopes of Sarojben’s recovery, but the doctors didn’t. Her family’s economic condition was not strong, but the doctors came forward to her support.
Dr. Apara put the efforts into the fund and collected around Rs. 80,000 for the treatment. Resident doctors also managed to get the very expensive Inj. Rituximab free-of-cost and GCS management also generously provided treatment and medicines at concessional rates. The patient was on a ventilator for 40 days with regular EEG monitoring.
The patient also underwent plasmapheresis under nephrologist guidance. Finally, treatment of Dr. Apara, Dr. Vipul, and Dr. Bhavesh Shah (Head – Critical Care) along with 24×7 supervision of the nursing staff and family’s prayers get fulfilled and Sarojben’s seizures were stopped with improvement in her health, and she was shifted to the ward from ICU after 53 days of combat for her health.
Sarojben’s condition is currently improving, and she will be discharged soon from the hospital.
